School-Based 

Support Worker

St. Peter in Chains is pleased to partner with Community First Solutions to offer a School-Based Support Worker (SBSW). This professional provides direct services to students and families with the aim of identifying and addressing barriers to school success and increasing the student's social-emotional skills.

The SBSW also facilitates coordination of care and refers students and families to appropriate service providers. In addition, our support worker can aid in crisis de-escalation and works closely with school staff to provide guidance and advice while maintaining student confidentiality.
